Types of Glaucoma: Chronic and Acute Glaucoma

Chronic or Open-Angle Glaucoma

Chronic glaucoma is the most common. It affects both eyes, often unevenly. One eye is less affected than the other and compensates for the loss of vision. It accounts for 80% to 90% of cases. This type of glaucoma is very progressive and asymptomatic. It may remain symptom-free for 10 to 20 years or longer. During this time, only a thorough eye examination can detect it.  Rarely, people may have dull pain (such as pressure in the eyes). Also, they may feel watery and have headaches. Visual symptoms take a long time to appear. The disease progresses slowly and steadily. The patient has vision loss at the periphery of the visual field. However, central vision (which enables people to read, write, and recognize faces) remains normal for a long time. This is called tubular vision (as if through a tube)

Acute Glaucoma or Angle-Closure Glaucoma

In the eye, there are two distinct chambers: the anterior and posterior segments.

The anterior segment is behind the cornea and includes the iris (the colored part of the eye) and the lens (the eye’s natural lens attached to the eyeball by ligaments, which separate the two chambers). It is filled with a clear liquid, the aqueous humor, which is produced by an organ called the ‘ciliary body’, located behind the iris, at the base of the ligaments of the lens. The back of the eye contains a kind of clear gel called the vitreous humor.

In glaucoma, the problem is in the anterior segment, where the pressure increases. The increased pressure is then transmitted indirectly to the posterior segment, and thus to the optic nerve.

Glaucoma is a Draining Problem

The aqueous humor is constantly renewed: it is produced by the ciliary bodies and discharges through a kind of filter called the trabecular meshwork, which is located at the angle at that the iris forms with the cornea. The aqueous humor is then directed to the venous system. In glaucoma with increased intraocular pressure, the aqueous humor has difficulty being eliminated. This problem of emptying increases the pressure in the anterior segment suddenly and dramatically in acute glaucoma, very gradually and less if glaucoma is chronic.

The increased pressure in the eye interferes with the functioning of the optic nerve and causes nerve fibers to degenerate. This destruction occurs very quickly when the pressure is very high (as in acute glaucoma), so medical treatment is urgently needed. In chronic glaucoma, IOP remains moderately elevated but, over time, this moderate increase causes loss of nerve fibers.
